This form is for MATH courses only. For other evaluations, like PHIL, PSYC, LING, MGMT, etc., please contact that department.

To have us evaluate (or pre-approve) non-UA math courses for UA equivalency credit, and/or to convert T-number credit into specific UA MATH credit (so that it can meet course requirements), please submit the following form.

Before completing this form, be sure you have read our information on transferring math credit.
For courses taken (or to be taken) at institutions inside Arizona, you should not need to complete this form unless it is for upper division coursework: you do not need pre-approval (instead, see the Arizona Transfer Course Equivalence Arizona Transfer Course Equivalence Guide), and the credit should automatically appear as UA MATH credit in UAccess Student.

With the online form, the evaluation process usually takes one to two weeks to complete. Incomplete forms will not be processed.

Non-UA math courses to be evaluated

You must complete a separate form for each academic institution. But for institutions in Arizona, there is no need to complete the form at all; instead, courses will transfer according to the Arizona Transfer Course Equivalence Guide. And for institutions outside the United States, you cannot use this online form and must instead download the paper form.

For pre-evaluation of courses you are considering taking, list the courses below. If you are unsure about which course to take, indicate which UA course you want to satisfy in the third column below, or type it in the Miscellaneous box at the bottom of this form. For evaluation and transfer of completed courses, list only those courses in which you obtained a grade of C or better.
If any of the courses being evaluated were taken online, provide a link to the syllabus for the online course. The online course must administer proctored exams in order to be considered as a substitute for a UA Math course. If a link is not available, email the syllabus to placement@math.arizona.edu. Include your course information, name, and UA SID on the email.
Finally, for evaluation or pre-evaluation of courses that will substitute for a MATH 302A/B requirement, you cannot use this online form and must instead download the paper form.
