Request Math Course Equivalency Evaluation or Credit

This form is for MATH courses only. For other evaluations, like PHIL, PSYC, LING, MGMT, etc., please contact that department.

To have us evaluate (or pre-approve) non-UA math courses for UA equivalency credit, and/or to convert T-number credit into specific UA MATH credit (so that it can meet course requirements), please submit the following form.

Before completing this form, be sure you have read our information on transferring math credit. (In particular, you must have official copies of your transcripts sent to the UA. Depending on the circumstances, the form below may furthermore instruct you to submit unofficial copies to the Math Placement Office.)
For courses taken (or to be taken) at institutions inside Arizona, you should not need to complete this form: you do not need pre-approval (instead, see the Arizona Transfer Course Equivalence Guide), and the credit should automatically appear as UA MATH credit in Student Link (at which time you may request a math placement review to obtain an updated math placement code). However, if the credit instead shows up as a T-number, or if you require math placement prior to the credit showing up anywhere in Student Link, please complete the Arizona version of this form.
For coursework to be used to meet MATH 302A/B requirements, do not submit the online form; instead, please download the Course Equivalency Verification, print it out, carefully follow the MATH 302A/B instructions, and submit the completed material to the Math Placement Office.
For courses taken (or to be taken) outside the United States, do not submit the online form; instead, please download the Course Equivalency Verification, print it out, carefully follow all instructions, and submit the completed material to the Math Placement Office.

With the online form, the evaluation process usually takes one to two weeks to complete. Incomplete forms will not be processed.
