If you have a math course that was taken outside of UA, do not submit this form. Go to the Transfer Evaluation Form.

If you are a freshman student who has not yet attended orientation, do not submit this form. Your placement will be updated during orientation. If you have any questions about your placement, go to math advising during your orientation session.

If you are being denied registration in a particular course due to an eligibility-related problem (“registration hold due to math placement denied”), yet believe you have gone through our placement process and met the requirements to obtain the necessary placement code, please complete this form to request a review of your record. The review is usually completed within one week of your request.
