You must complete a separate form for each academic institution. But for institutions in Arizona, there is no need to complete the form at all; instead, courses will transfer according to the Arizona Transfer Course Equivalence Guide. And for institutions outside the United States, you cannot use this online form and must instead download the paper form.

For pre-evaluation of courses you are considering taking, list the courses below. For evaluation and transfer of completed courses, list only those courses in which you obtained a grade of C or better. Finally, for evaluation or pre-evaluation of courses that will substitute for a MATH 302A/B requirement, you cannot use this online form and must instead download the paper form.
